PicoSure® Laser Tattoo Removal

Tattoos are incredibly popular, and offer a way for individuals to express themselves, their feelings, and their passions. However, as the number of people getting tattoos increases, so does the number of people looking to have their ink removed. Feelings and passions change, but a change of heart does not have to be a lifelong burden.

Montilla Plastic Surgery is proud to offer PicoSure® treatments, which has one of the most advanced lasers designed to get rid of tattoo ink for good. PicoSure® uses ultra-short pulses, measured in billionths of a second in length, to hit the ink with tremendous pressure. As the laser targets the dark pigment, it shatters that pigment, breaking it into tiny particles. These ink bits are then metabolized by your white blood cells for processing out of the body. This safe and effective laser will work on the full spectrum colored tattoo inks, including black, blue, green, red, orange, purple, yellow and pink.

After Your PicoSure® Tattoo Removal Session

Recovering from a PicoSure® tattoo removal session will take a few days, and healing normally occurs in two to three weeks.

The initial 24 to 48 hours may involve some discomfort, and the treatment area will feel like a mild to severe sunburn. Blisters are not uncommon and should be left alone to resolve on their own.

During the first several days, patients should avoid any vigorous activity that may irritate the treatment area. To prevent rubbing, loose clothing should be worn.

The treatment area should not be exposed to the sun, as this could result in discoloration. It can take a week or more for the blisters to dry up and peel.

Ice may be applied for 20 minutes every hour during the first day, then every two hours. A sterile dressing, Tegaderm, provided by our office, should be worn for the first three to five days. The Tegaderm plus the ice will help to decrease the pain.

Dr. Montilla recommends Silvadene® cream be applied twice daily once the Tegaderm is removed. You will be given a prescription.

PicoSure® Skin Rejuvenation Treatments

The PicoSure® laser can also be used for rejuvenating the face, hands, legs, décolleté, chest and back. It can help with acne scars, pigmented lesions and some fine lines. The PicoSure® is perfect for those looking to improve the skin’s texture and tone and there is little or no down time.


Before Your PicoSure® Skin Rejuvenation Session

All laser tattoo removal are performed in our Boston and Worcester office and typically take less than 15 minutes per session.

Patients should wear loose-fitting, comfortable clothing, especially over the target area.

While the PicoSure® laser is less painful than other techniques, there is still a degree of discomfort involved with tattoo removal, while fractional treatments for rejuvenation have minimal pain.

A topical numbing cream will be applied 30-45 minutes before the treatment. Ice is also applied after treatment. On occasion, Dr. Montilla will inject lidocaine in selected areas if needed for comfort.

Potential Risks of PicoSure®

While scarring is possible with any laser tattoo removal, it is rare, especially when the treatment is performed by an experienced professional. Dr. Montilla or a laser technician will conservatively treat the pigmented area in a meticulous and precise manner to avoid scarring. This also decreases the chance for hypopigmentation, which can occur in patients with darker skin or having dark tattoo inks removed. During your tattoo removal consultation with Dr. Montilla, he will review all potential risks and complications.

Cost of a PicoSure® Treatment

The total cost of laser tattoo removal in Boston will depend on the individual patient’s tattoo. The size, type of ink, and age of the tattoo can all factor into the number of treatments that may be required. On average, PicoSure® treatments start at $100 - $250 each for a business card size or smaller. A tattoo that is the size of two business cards is usually about $500 per session. The advanced PicoSure® technology means that fewer treatments are required, sometimes cutting the total number in half. There is no additional charge to treat more than one color of ink. We will give you a quote for larger tattoos at your consultation. There are savings on offer when purchasing four or more treatments as a package.

A PicoSure® consultation appointment with Dr. Montilla is $100. If you are treated the same day, this amount will be deducted from the cost. At the consultation appointment, Dr. Montilla or the laser technician will look over your tattoo(s) and discuss your desired results. They will then explain all of your tattoo removal options and show before/after photographs of his recent patients. If it is decided that the PicoSure®tattoo removal process in Worcester is right for you, a personalized treatment plan will be created, and the total cost of treatment will be established.

Although the PicoSure® has the ability to address all colors of tattoo ink at a faster rate than other lasers, multiple sessions will help attain the optimal, desired results. The number of sessions required is related to the colors used, the depth of color, the size of the tattoo, and its location. Most patients should expect a series of four to six treatments, spaced at least for six to eight weeks apart. This treatment is also useful for skin rejuvenation of scars, wrinkles and sunspot removal and can be done in our Boston and Worcester office, a series of four to six treatments is also standard.

PicoSure® FAQ's

How long will a session with the PicoSure® take?

Depending on the size of the tattoo being treated, a small PicoSure® session normally takes less than 40 minutes.

How many treatments does the PicoSure® tattoo removal require?

Depending on the individual tattoo, four to six PicoSure® sessions are typical to receive optimal results. However, some tattoos may require more treatments.

How frequently will I need to come for repeat sessions?

Dr. Montilla recommends waiting between four and six to eight weeks between PicoSure® tattoo removal sessions, allowing the body time to break down and eliminate the excess ink.

What color ink can the PicoSure® treat?

The PicoSure® can successfully treat any color of tattoo ink, including red, blue, green, orange, yellow and black. Yellow is one of the more difficult colors to remove.

Will the PicoSure® fully eradicate my tattoo?

Yes, most tattoos can be completely removed using the PicoSure®, however in some cases, there may be residual pigmentation.

Will the PicoSure® cause scarring?

Treatments with the PicoSure® do not normally result in scars. Montilla Plastic Surgery will discuss the risk of scarring during the consultation appointment.

What are the potential risks of tattoo removal with the PicoSure®?

The PicoSure® is considered safe, with little chance for complications. However, like other laser treatments, there is always the small risk of blistering, infection, scarring, and hypo-pigmentation or hyper-pigmentation.

Will my PicoSure® tattoo removal hurt?

Unfortunately, the tattoo removal process is uncomfortable. The PicoSure® provides a faster, less painful treatment than other lasers. Additionally, the PicoSure® requires fewer sessions, making the removal process quicker.

Practice Policy Update Regarding COVID-19

Dear Patients & Visitors

To limit the spread of COVID-19 and to keep the community and our patients healthy, Montilla Plastic Surgery is in compliance with CHCs guidance for the Commonwealth's Reopening website. Please see our Phase 1 Reopening Attestation.

In addition to the CHC guidelines, we are taking the following precautionary measures effective immediately:

  • All patients are asked to contact our office 508-948-4030 at least 24 hours prior to their appointment.
  • Any patient with symptoms of illness such as cough, fever, difficulty breathing, or upper respiratory symptoms must reschedule their appointment by calling 508-948-4030.
  • Notify staff immediately if you have traveled or have been in potential contact with a person known to have or suspected to have COVID-19.
  • At this time, patients may schedule an appointment in the office, and Dr. Montilla is available for telemedicine consultations and follow-ups as well.
  • Only patients will be allowed in the office and the exam rooms. Individuals accompanying the patients will be asked to wait in their cars. The only exceptions will be:
    • Patients who are minors (only one parent allowed to accompany the child)
    • Interpreters
    • Caretakers for Non-ambulatory or informed patients requiring assistance (only one allowed)
  • Governor Baker has put a mandatory moratorium on all elective surgeries beginning Wednesday, March 18th and continuing for at least the next two to four weeks.
  • All employees at Montilla Plastic Surgery will adhere to the Center for Disease Control and Prevention (CDC) and Massachusetts Department of Health and Human Services community mitigation strategies related to COVID-19.
  • CDC has identified older adults and those who have serious chronic medical conditions as high risk for getting very sick from COVID-19. Please notify our staff immediately if you wish to reschedule your appointment to a later date.
